Bandhan Bank Visa Platinum Debit Card - Claims Procedure
Listed below are the process details for claiming Personal Accidental Insurance, Purchase Protection and Lost Cards Liability Cover.
Personal Accident (Death)
Upon occurrence of such an event, below information should be submitted to the respective branch not later than 30 days from date of incident:
- Card number
- Card variant
- Linked CASA number
- Deceased name
- Date of death
- Died due to (Rail/Road/Air)
- Brief fact of the case
- Nominee address and contact number
Please submit a hard copy of the below-mentioned documents to your branch within 60 days of reporting the claim:
- The claim form duly filled and signed by the claimant
- Death certificate notarised/attested by a gazetted officer
- FIR notarised/attested by a gazetted officer
- Post Mortem Report notarised/attested by a gazetted officer
- Copy of all treatment papers, if deceased was hospitalised
- Copy of PAN card, residence proof and 2 passport size colour photos of claimant
- Cancel cheque with NEFT mandate form, dully filled in by the claimant and bank
- Duly filled C-KYC form
- NOC from the claimant
- Additional documents may be requested based on merit of case
Lost Card Liability, Skimming, Online Fraud Protection & Purchase Protection
Upon occurrence of such an event, below information should be submitted to the respective branch not later than 30 days from date of incident:
- Card number
- Card variant
- Linked CASA number
- Date of Incident
- Loss Amount
- Card Blockage Date
- Transaction done by (IMPS/UPI/Card/POS)
- Brief fact of the case
Please submit a hard copy of the below-mentioned documents to your branch within 60 days of reporting the claim:
- Claim form duly filled in and signed by the claimant: original
- Latest account statement (for the month of loss)
- Incident report by the Bank
- Copy of dispute letter given by customer to the Bank
- Cancelled cheque of the claimant OR NEFT mandate form duly filled and verified by the Bank (attached)
- Certified copy of FIR
- Additional documents may be requested based on its merit
Note:
Please intimate well within 30 days from the date of incidence, to allow insurance company to investigate the loss at an initial stage. Also, all documents as mentioned above should be submitted within 60 days from the date of reporting of claim to Bandhan Bank.
