Bandhan Bank Mastercard Platinum Plus Debit Card - Claims Procedure

Listed below are the details of the process for claiming Personal Accidental Insurance, Purchase Protection, and Lost Card Liability Cover.

 

Personal Accident (Death)
 

Upon the occurrence of such an event, the following information should be submitted to the respective branch head no later than 30 days from the date of the incident:

  • Card number
  • Card variant
  • Deceased name
  • Date of death
  • Died due to (rail/road/air)
  • Brief fact of the case
  • Nominee address and contact number
     

Please submit a hard copy of the documents mentioned below to your branch within 60 days of reporting the claim:

  • The claim form duly filled and signed by the claimant
  • Death certificate notarised/attested by a gazetted officer
  • FIR notarised/attested by a gazetted officer
  • Post Mortem Report notarised/attested by a gazetted officer
  • Copy of all treatment papers, if deceased was hospitalised
  • Copy of PAN card, residence proof and 2 passport size colour photos of claimant
  • Cancel cheque with NEFT mandate form, dully filled in by the claimant and bank
  • Duly filled C-KYC form
  • Additional documents may be requested based on merit of case

 

Lost Card Liability, Skimming, Online Fraud Protection & Purchase Protection
 

Upon occurrence of such an event, below information should be submitted to the respective branch head not later than 20 days from date of incident:

  • Card number
  • Card variant
  • Cardholder name
  • Date of Incident
  • Card blockage date and time
  • Loss amount
  • Brief fact of the incident
     

Please submit a hard copy of the below-mentioned documents to your branch within 60 days of reporting the claim:

  • Claim form duly filled in and signed by the claimant: original
  • Latest account statement (for the month of loss)
  • Complete passport copy, if loss is at any international location
  • Incident report by the Bank
  • Copy of dispute letter given by customer to the Bank
  • AML documents if claim amount more than ₹1,00,000 claimed on an individual's account (Two passport size photograph, ID and residence proof of the claimant, as per the list provided in last section of the claim form)
  • Cancelled cheque of the claimant OR NEFT mandate form duly filled and verified by the Bank.
  • Proof of remittance that the Bank has made good the loss
  • Investigation report
  • Certified copy of FIR
